Idarubicin hydrochloride

Idarubicin is an antimitotic and cytostatic anthraycline. Idarubicin intercalates with DNA and interacts with topoisomerase II and has an inhibitory effect on nucleic acid synthesis.

Indications: Acute nono-Iymphocytic leukemia, advanced breast cancer.

Dosage of Idarubicin hydrochloride

30mg/m2 orally given daily for 3 days as a single agent, or between 15 to 30mg/m2 orally daily for 3 days in combination with other antileukemic agents. In advanced breast cancer 45mg/m2 orally given either on a single day or divided over 3 consecutive day (15mg/m2/day) to be repeated every 3 or 4 weeks based on the haematological recovery. A dose of 35 mg/m2 orally on one day may be administered in combination with other cytotoxic agents. In patients with hepatic impairment a dose reduction of it should be considered.

Contra-Indications of Idarubicin hydrochloride

• Hypersensitivity, severe renal and liver impairment or patients with uncontrolled infections.
• Pregnancy and lactation.

Special Precautions of Idarubicin hydrochloride

• Pre-existing bone marrow depression,
• pre-existing heart disese,
• in patients with prior total body irradiation or bone marrow transplantation.
• Toxic effects on the myocardium, represented by congestive cardiac insufficiency, acute arrythmias or other cardiomyopathies,
• Potentially lethal may occur during therapy or some weeks after the end of therapy.
• Since hepatic function impairment can affect the bioavailability of idarubicin,
• liver function should be evaluated.

Side effects of Idarubicin hydrochloride

• Severe myelosuppression and cardiotoxicity are the two major adverse effects,
• alopecia, reversible in most of the patients acute nausea and vomiting,
• mucositis,
• esophagitis,
• diarrhoea,
• fever,
• tremors,
• cutaneous rash,
• increase in liver enzyme.
